Differentiated Curriculum Samples is part of the “Reaching All Students” site, which is sponsored by the Oregon Department of Education Gifted and Talented Office.  On the Differentiated Curriculum Samples main page is a chart that lists the name of the activity, the targeted grade level and the subject area.  The downloadable lessons are beautifully done and contain all the resources you need —  including templates and graphic organizers —  for differentiating the lesson for gifted students.  The lessons are connected to the Oregon state standards, but could be adapted to fit the standards of other states.
